Etoposide or VP-16 (trade name Etopophos) is a cytotoxic anticancer drug which belongs to the topoisomerase inhibitor drug class. It is used in form of its salt etoposide phosphate.
It is on the WHO Model List of Essential Medicines, the most important medications needed in a basic health system.

Medical uses:

Etoposide is used as a form of chemotherapy for cancers such as Kaposi’s sarcoma, Ewing's sarcoma, lung cancer, testicular cancer, lymphoma, nonlymphocytic leukemia, and glioblastoma multiforme. It is often given in combination with other drugs (such as bleomycin in treating testicular cancer). It is also sometimes used in a conditioning regimen prior to a bone marrow or blood stem cell transplant.

Administration:

It is given intravenously (IV) or orally in capsule form. If the drug is given IV, it must be done slowly over a 30- to 60-minute period because it can lower blood pressure as it is being administered. Blood pressure is checked often during infusing, with the speed of administration adjusted accordingly.

In a randomized, double-blind, phase II trial combining sorafenib with doxorubicin, the median time to progression was not significantly delayed compared with doxorubicin alone in patients with advanced hepatocellular carcinoma. Median durations of overall survival and progression-free survival were significantly longer in patients receiving sorafenib plus doxorubicin than in those receiving doxorubicin alone.A prospective single-centre phase II study which included the patients with unresectable hepatocellular carcinoma (HCC)concluding that the combination of sorafenib and DEB-TACE in patients with unresectable HCC is well tolerated and safe, with most toxicities related to sorafenib.This is the only indication for which sorafenib is listed on the PBS and hence the only Government-subsidised indication for sorafenib in Australia. Along with renal cell carcinoma, hepatocellular carcinoma is one of the TGA-labelled indications for sorafenib

Mechanism of action

Etoposide forms a ternary complex with DNA and the topoisomerase II enzyme (which aids in DNA unwinding), prevents re-ligation of the DNA strands, and by doing so causes DNA strands to break.Cancer cells rely on this enzyme more than healthy cells, since they divide more rapidly. Therefore, this causes errors in DNA synthesis and promotes apoptosis of the cancer cell.
Nadir: Meaning low point, nadir is the point in time between chemotherapy cycles in which you experience low blood counts.

Hair loss
Menopause (chemotherapy induced)
Loss of fertility. Meaning, your ability to conceive a child may be affected by etoposide. Discuss this issue with your health care provider.
Nausea and vomiting (especially at high-doses)
Low blood pressure (if the drug is infused too fast)
These side effects are less common, meaning they occur in 10-29 percent of patients receiving etoposide:

Mouth sores (especially at high doses)
Diarrhea (especially at high doses)
Poor appetite
Radiation recall (see skin reactions)
Other side effects:

Metallic taste during infusion of drug
Inflammation at injection site
Peripheral neuropathy (numbness in your fingers and toes) may occur with repeated doses. This is a rare side effect but can be irreversible. Report numbness or tingling of feet or hands to your health care provider.
